Take the First Step Towards Specialized Care and Lasting Wellness

I’m glad you’re here. Starting care is an important step. The intake process has been thoughtfully designed to feel clear and supportive from the beginning.

What to Expect:

1. Choose a 90-minute appointment time that works for your schedule. If you don’t see any times that work for you, please reach out and we can discuss how we may be able to accommodate your schedule.

2. Enter Your Payment Information: A credit or debit card is required when booking to secure your appointment time. Your card will not be charged at the time of booking; payment will be processed 24 hours prior to your scheduled appointment time.

3. After booking, you’ll receive a brief intake questionnaire by email. It should take no more than 10 minutes to complete and will help me get a sense of what you’re looking for and how I can best support you. You’re welcome to leave any questions blank or mark them N/A if they don’t apply to you.

4. After completing the questionnaire, you’ll review and electronically sign a few standard consent forms that provide important information about the practice and your care. If you would like to review the consent forms before booking, please reach out and I’ll be happy to provide them. Please complete the questionnaire and consent forms at least 48 hours prior to your appointment, to give me adequate time to review and prepare for your visit.

Before Your Appointment:

Once scheduled, you'll receive a confirmation email with details about your visit and the link to join the video-chat. If anything feels unclear along the way, please feel free to reach out. All intake registration is handled through a secure platform to protect your privacy.

Not Sure if We're a Good Fit?

If you're interested in starting care but aren't quite ready to book an appointment, you're welcome to schedule a complimentary 15-minute introductory consultation. This call is an opportunity for prospective patients to ask questions, discuss their goals, and determine whether my practice is a good fit for their needs. Please note that no formal clinical assessment, diagnosis, or treatment recommendations will be provided during this call.

Outpatient Psychiatric Care for Adults in Utah

Comprehensive assessment, diagnosis, and medication management, delivered remotely with the time and attention you deserve.

Initial Evaluation:

Your first appointment is 90 minutes, giving us time to explore your history, understand your symptoms and concerns, and develop a clear clinical picture before discussing any treatment recommendations. Our conversation is guided and collaborative—you don't need to prepare or know exactly what to say. I'll ask structured questions about your symptoms, medical history, and life experiences while also giving you plenty of space to share what's most important to you. If you're unsure where to begin, I'll guide you through the process, and you're always welcome to ask questions or bring up anything that's on your mind along the way.

We'll complete a comprehensive assessment, clarify your diagnosis, and review your treatment options in detail. If medication is appropriate, we'll discuss the clinical reasoning, risks and benefits, and what to expect before anything is prescribed.

You'll leave with a clear understanding of your diagnosis, an individualized treatment plan, and a follow-up timeline tailored to your needs. You're also welcome to reach out between visits if questions or concerns come up.

Fee: $300 | Duration: 90 minutes

Ongoing Treatment:

Follow-up visits are 30 minutes, twice what most psychiatric practices offer, with time to review how your medication is working, address concerns, and adjust your plan as needed.

Visit frequency adapts to where you are in treatment: every one to two weeks during active medication changes, extending to every 8–12 weeks once you feel stable. Sooner appointments are always available if something comes up.

Fee: $150 | Duration: 30 minutes

Areas of Practice:

Sage Meadows treats adults (18+) experiencing a wide range of psychiatric conditions, including major depressive disorder, generalized anxiety disorder, panic disorder, OCD, PTSD, bipolar disorder (type I and II), ADHD, autism spectrum disorder, schizoaffective disorder, and schizophrenia.

This list is representative. If you're experiencing symptoms that concern you, I encourage you to reach out even without a formal diagnosis.
